Our institutional recent outcomes of liver tranplantation





  • Thrombotic microangiopathy (TMA) is one of the important complications occurring after liver transplantation (LT), and it is suggested that a Von Willebrand factor (VWF) and ADAMTS13 may play an important role in the onset of TMA, causing poor outcome after LT. 
  • We measured the ADAMTS13 activity, VWF propeptide (VWFpp) and VWF antigen in the plasma during living donor LT (LDLT) in order to examine their usefulness in the diagnosis of TMA after LT. 
  • In this study, VWFpp and ADAMTS13 play an important role in the onset of TMA after LDLT and related to the outcome. It is suggested that the VWFpp may be a more useful marker for the diagnosis of TMA than VWF.





Can serum Tenascin-C be used as a marker of prognosis in patients after living donor liver transplantation?

  • Tenascin-C (TN-C), an extracellular matrix glycoprotein, is not normally expressed in the liver but transiently reappears under various pathologic conditions to play important roles in tissue remodeling during inflammations such as chronic HCV hepatitis.  
  • Elevated serum TN-C levels in those patients were reported to reflect the tissue injury and remodeling.
  • It is therefore considered that serum TN-C could be effective as a marker of liver ischemia reperfusion injury and regeneration insufficiency in living donor liver transplantation (LDLT).








Conclusion

  • Serum TNC14 can be used as the predictive marker of prognosis in patients who underwent LDLT. 
  • Especially, if graft size is smaller, serum TNC14 is potentially useful for early diagnosis of postoperative liver regeneration impairment.
